Medical Form/Blanket Permission Form. In addition, the Medical Release Form was handed out separately (it is green) and needs to be notarized. Notary service is available in many places, such as grocery stores, banks, post offices, our own main office here at East Cobb, and many office places. Download 0910 Medical Form
Band Uniform. All band students will need to wear the East Cobb Band Shirt for performances, which is a navy blue polo shirt with the ECMS Band logo. Order forms were made available to all students during the first two weeks of school and are available in the band room. Forms need to be turned in to the white box in the front of the band room and payment needs to be placed in the school money envelope and placed in the safe next to the Media Center. 7th & 8th graders will have additional requirements for the LGPE performances, which are listed in the orange syllabus packet. Lockers. We are fortunate here at East Cobb to have a large selection of instrument lockers for students to use, and many students will be using lockers this year to store their instruments. The locker form is light blue and needs to be signed and returned before a student will be issued a locker. Here is the locker form: Download Locker Policy 0910
School-owned instruments. We are aware that situations arise and students need to use a school-owned instrument. Sometimes the reasons are financial in nature; sometimes the student's choice of instrument is more conducive for using a school-owned instrument. Before we issue school-owned instruments, we would like for you as parents to be as educated as possible regarding the costs of renting/purchasing an instrument. Please contact Mrs. Holbrook for the necessary forms to start this process. Please be aware that we do NOT have any school-owned alto saxophones or percussion kits. Also, please note that if your student is issued a school-owned instrument, you will still need to provide your child the appropriate book and other accessories that he/she needs in order to play his/her instrument.
